New York Connects

March 29 at the Hadley-Luzerne Public Library from 6:00-7:00 pm.

Susan Dornan, NY Connects Coordinator of Warren-Hamilton Counties Office for the Aging will provide an overview on long term care services and supports available for people of all ages.  She will provide information about NY Connects, Long Term Care Council, Office for the Aging, Medicare’s Savings Program and free preventive services, home care, transportation and more.  You will have an opportunity to ask questions about long term care services and supports available.  She will also have information on the new Caregiver Support Initiative through SUNY Plattsburgh Center for Neurobehavioral Health that offers paid respite, support and much more.  If you are not able to attend and have questions, please call 1-866-805-3931.
